OBJECTIVES. Mandibular overdentures supported by four miniscrew implants represent a satisfactory solution for those patients where fixed prosthesis are impossible because of anatomical limitations. Surgical trauma is very light, especially with a flapless technique, that is without tissue incisions and sutures. MATERIALS AND METHODS. In all patients four monophasic implants with a 2,5 mm wide ball attachement were inserted among mandibular foramina. The overdentures were applied in the same day following the immediate loading protocol. In each patient an implant was removed: three implants were removed within four weeks, while the fourth was removed after three years because of fracture. All samples were sent to the D'Annunzio University laboratory in Chieti for histological examination. RESULTS AND CONCLUSIONS. Histological examination of implants removed in the first month showed that bone remodeling takes place after fixture insertion while osteointegration is in progress; histological examination of the fourth implant removed after three years showed a perfect osteointegration.
